Due to continued success and expansion, Winchester City Primary Care Network (PCN) is looking for an enthusiastic Care Coordinator to join our friendly and supportive team.
You'll work with our three GP practices in central Winchester, helping patients access the right care and support while ensuring clinicians can focus on what matters most, delivering high-quality care.
You'll work alongside our experienced Care Coordinators and report to the Multi Service Manager, supporting effective workload management and helping clinicians focus on patient care.
What you'll do
Work closely with GPs, nurses, and other health professionals to coordinate patient care.
Liaise with hospitals, community services, and external agencies to ensure smooth communication and continuity of care.
Provide essential admin and organisational support, including processing referrals, managing records, and supporting meetings.
Contribute to projects improving population health across Winchester.
Ensure timely, accurate record keeping and maintain confidentiality in line with data protection standards.
Collaborate with Care Coordinators across the PCN to share learning and provide service cover when required.

Confident using Microsoft Office and clinical systems such as EMIS Web (training can be provided for EMIS Web)* Keen to learn and develop in a dynamic primary care environment.

Job Types: Part-time, Permanent
Pay: 13.00-14.07 per hour
Expected hours: 25 - 30 per week
