Overview:
Aplos Health PCN was founded in 2019 and consists four practices in the London Borough of Lewisham -
Sydenham Green Group Practice, The Vale Medical Centre, Wells Park Practice and Woolstone Medical Centre.
We provide care to a patient population of 50,000. In a little over a year and despite a global pandemic we have developed excellent working relationships between the practices. We have run educational events for patients, started an adolescent and contraceptive clinic and are now looking forward to developing further innovative pathways and services for our patients.
We are seeking an experienced, self-motivated, hard-working, and patient focused healthcare assistant to join our PCN. They will work closely with the GPs and other primary care professionals within the PCN to proactively identify and manage a caseload of patients, making sure that holistic support and is made available to them and that their complex needs are addressed. They may also be asked to contribute towards other administrate tasks within the GP Practice or PCN.
:
1. Proactively manages patient call and recall system for those with complex care needs.
2. Responds to all queries and requests for assistance from patients within care target group in regard to their care plan and all aspects of their clinical management. Signpost patients and their carers to appropriate support both inside and outside the practice.
3. Monitors delivery of care by completing patient meetings; documenting care; identifying progress toward desired care outcomes.
4. Reviews the care plan with patients interacting with involved departments to negotiate and expedite care where possible.
5. Works with clinicians to ensure all the contractual requirements for patients with complex care needs are met.
6. Manages referrals and provides information/ advice for a range of different community healthcare services.
7. Assists in the administration and coordination of MDT meetings, ensuring team recommendations are completed in a timely manner.
Miscellaneous
Assist with the gathering of statistics and information when required, as requested by Health Professionals and Practice & PCN Management
Provide cover for members of the administration and reception team during periods of sickness and annual leave.
Assist in the practice, PCN and borough wide covid pandemic response as required o Providing organizational and operational support for the setup of COVID vaccination clinics
Any other delegated duties considered appropriate to the post Clinical System /
Medical Records:
Familiarity with the clinical system
Book appointments and recalls ensuring sufficient information is recorded
Extraction of necessary data from clinical correspondence and input into Electronic Patient Record, ie. GP2GP and Docman system
Ensure correspondence, reports, results etc. are dealt with in a timely manner as per practice protocol
Responsibility for target group disease register
Utilise social prescribing template on EMIS to ensure data quality Confidentiality: In the performance of the duties outlined in this , the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers.
They may also have access to information relating to the Practice and PCN as a business organisation. All such information from any source is to be regarded as strictly confidential.
