Job Description

Key Responsibilities:



Care Coordination & Leadership:

Create, manage, and maintain efficient staff rotas for all clients, ensuring all care visits are covered seamlessly.

Act as the main point of contact for our team of care assistants, providing them with daily support, guidance, and leadership.

Assist with recruiting, inducting, and mentoring new care staff, fostering a supportive and skilled team environment.

Liaise confidently with health professionals (e.g., District Nurses, GPs) and family members to ensure a joined-up approach to care.

Hands-On Community Care:

Conduct initial assessments and onboarding for new residents/clients, ensuring a warm welcome and a smooth start to their care journey.

Provide hands-on personal care and support to clients in the community to cover visits, build relationships, and lead by example.

Perform regular spot checks in clients' homes to monitor the quality of care provided.

On-Call & Operational Duty:

Participate in the on-call rota to provide urgent support and advice to care staff and clients outside of office hours.

Accurately maintain and update client care records, ensuring compliance with CQC standards.

Essential Requirements:

Must have a recognised Level 3 Diploma in Health and Social Care (or higher).

A minimum of 2 years of experience in a home care setting within the UK.

Proven experience in providing hands-on personal care is essential.

Full UK driving licence and access to your own vehicle.

Exceptional organisational and communication skills.
