As a Care Coordinator under the CPA, you'll manage a defined caseload, assess, plan, and implement person-centred care, and contribute to the ongoing development of the service, particularly its training provision. You'll play a key role in promoting autonomy, hope, and community integration for those you support. What you can add:
You will undertake comprehensive assessments of clients' needs and strengths, including risk assessments, and plan, implement, and evaluate care collaboratively with clients, the multidisciplinary team, and other agencies.
You will work flexibly to meet the evolving needs of clients and the service, supporting individuals to achieve their fullest potential through a recovery-focused approach.
You will be encouraged to continuously develop your professional skills and knowledge, actively contributing to service excellence while benefiting from regular supervision and appraisal.
You will play a key role in coordinating and participating in CPA meetings, ensuring that all care, crisis, and contingency plans are thoroughly documented and shared via the ePJS system.
You will provide cover for colleagues during leave periods and participate in duty rotations as required.
What you'll need:
Registered Mental Health Nurse/Social Worker/Approved Mental Health Professional/Occupational Therapist
Mentorship qualification and post-qualification training
Experience in community mental health, including work with clients with enduring mental health issues, psychosis, and challenging behaviours
UK NHS Experience
Proven collaboration with external agencies and multi-disciplinary teams
Experience working with families and in residential settings
Desirable:
Access to a car and UK drivers licence
