Join our passionate and supportive team at the Croydon Community Mental Health Team for Older Adults, based at Heavers Resource Centre in Selhurst. We're looking for a compassionate and skilled mental health professional to coordinate care for older adults living in the community, supporting individuals with a wide range of mental health conditions including dementia, mood disorders, and complex needs.

As a Care Co-ordinator, you'll play a key role in delivering personalised, recovery-focused support through the Care Programme Approach (CPA). You'll work closely with a multidisciplinary team, visit service users in their homes, and build meaningful relationships with individuals, families, and carers to ensure holistic, person-centred care.

This is an excellent opportunity to grow within a dynamic and experienced team. You'll be supported to develop your skills and explore specialist interests such as psychosocial interventions, dual diagnosis, and physical health care, all while making a real difference in the lives of older adults in our community.

What you can add:

You will lead on personalised care coordination, managing a caseload under the Care Programme Approach (CPA), engaging service users with compassion, flexibility, and a focus on recovery and inclusion. You will deliver expert assessment and interventions, including triage, crisis response, and evidence-based therapies, while applying knowledge of key legislation such as the Mental Health Act and Care Act. You will assess and manage risk proactively, conducting regular risk reviews, responding to safeguarding concerns, and working collaboratively to promote safety in community settings. You will support families and carers, involving them in care planning, assessing their needs, and managing complex dynamics with sensitivity and respect. You will work collaboratively within a multidisciplinary team, contributing your professional expertise, maintaining clear communication, supporting service development, and championing ongoing learning and quality care.
What you'll need:

Recognised qualification and registration as a mental health nurse, occupational therapist, or social worker (NMC, HCPC, or Social Work England). Evidence of ongoing professional development. Clinical experience in community mental health and/or with older adults and their carers. Strong skills in clinical and mental health assessments. Experience in risk assessment and management. Ability to work in multidisciplinary teams. Experience collaborating with statutory and voluntary services.
Desirable:

Training or education related to the care of older people. Full driving licence and access to a car.
