We are looking for an enthusiastic Care Coordinator, CPNs, OTs, and Social Workers are all welcome to apply, who is eager to develop their skills in First Episode Psychosis care. In this role, you'll work closely with patients to create tailored care plans, support their recovery, and connect them to essential services, while collaborating with a dynamic team to ensure high-quality, holistic care. If you are passionate about making a real difference in people's lives, this is the role for you.

What you can add:

You will support patients in building skills to reduce relapse risk and identify early warning signs for timely intervention. You will implement a range of interventions to help patients understand their psychotic experiences. You will provide regular updates to the Multidisciplinary Team and the patient's GP regarding patient progress. You will work with patients to create comprehensive care packages that address all their needs. You will reflect on your practice through regular supervision and annual appraisals. You will actively engage in training and knowledge-sharing opportunities.
What you'll need:

Qualified RMN, OT or Social worker with current aligned professional registration (i.e. NMC, HCPC) Evidence of continuing professional development e.g. relevant training Significant post registration experience in a mental health setting Experience of working in a community setting Experience of working with service users with psychosis Experience of the use of evidence based interventions relating to the care of service users with psychosis
