Job Description

Liaison Between Clients and Staff Team - Care Support Workers / Volunteers



Manage client referrals, waiting lists, and initial assessments with discretion and efficiency

Conduct client visits, spot checks, risk assessments, reassessments, and care planning

Prepare, implement, and regularly review / amend / update care plans and MAR charts to ensure accuracy and responsiveness

Ensure communication sheets are read, actioned, and stored in line with compliance standards

Maintain and update digital systems (e.g. Webroster) to reflect current schedules and care plans

Match client needs with CSW skills and availability to ensure safe, high-quality support

Coordinate rotas, manage cancellations and changes, and ensure clear communication with all parties

Gather feedback from clients, carers, and staff, and collaborate with Senior CSWs and Project Leads to enhance service quality

Make referrals to external agencies where appropriate, ensuring continuity of care

Identify and escalate concerns, particularly those relating to health, safety, or safeguarding

Operational Oversight



Ensure care is delivered efficiently and within commissioned / allocated hours and budget

Ensure all care documentation--including care plans, MAR charts, and communication records--is accurate, up-to-date, and compliant with regulatory standards

Support the team in maintaining clear and timely care notes and incident reports

Facilitate information sharing across the team, including updates on safety protocols, care plans, and emergency procedures

Uphold confidentiality, safeguarding principles, and equal opportunity standards in all aspects of service delivery

Care Staff Management



Support recruitment, induction, supervision, and appraisal Contribute to team meetings and staff development

Quality and Compliance



Ensure regulatory compliance Support safeguarding, confidentiality, and equal opportunity principles

Carer Engagement



Consult with carers to assess satisfaction and future needs Represent carers' views in appropriate forums

Service Development



Feed community needs into business planning Identify staff training needs and contribute to the annual training plan

Promoting Crossroads Care



Represent the organisation locally and nationally Support presentations and outreach to local groups

General Duties



Uphold the Crossroads Care Code of Conduct Ensure compliance with health and safety legislation Work within the philosophy and policies of Crossroads Care Promote dignity, choice, and inclusion Undertake any other duties appropriate to the role

About You



You bring empathy, discretion, and a proactive approach to problem-solving You're confident managing multiple priorities and communicating with diverse stakeholders Experience in health, social care, or community support is desirable You're IT literate and comfortable with record-keeping and reporting You value emotional fairness, clear boundaries, and collaborative teamwork
Job Types: Full-time, Permanent

Pay: 24,000.00-30,000.00 per year
