We are currently seeking an
experienced Care Coordinator
to join our growing team. The successful candidate will play a key role in ensuring the smooth day-to-day coordination of care services, supporting both clients and care staff to maintain exceptional standards of service delivery.
Key Responsibilities
Coordinate daily care visits and rotas, ensuring efficient allocation of carers to meet client needs.
Maintain regular communication with clients, families, and care teams to ensure satisfaction and continuity of care.
Manage care schedules, ensuring all calls are covered and changes are communicated promptly.
Support the recruitment, induction, and supervision of care staff as required.
Participate in on-call duties on a rota basis to provide out-of-hours support.
Maintain accurate and up-to-date care records in line with CQC and company standards.
Assist with care assessments, care planning, and risk assessments.
Ensure compliance with company policies, health and safety, and safeguarding procedures.
Work closely with management to identify areas for service improvement and growth.
Requirements
Previous experience as a Care Coordinator
within domiciliary or community care is essential.
Strong organisational and time-management skills.
Excellent communication and interpersonal abilities.
Good IT literacy and experience using care management software.
A proactive and solution-focused approach.
Understanding of CQC regulations and compliance requirements.
A valid UK driving licence and access to your own vehicle (preferred).
