We are seeking for a dedicated and skilled Care Coordinator to join our healthcare team. The successful candidate will play a vital role in ensuring seamless communication and coordination of care services for our clients. Duties
Act as the primary point of contact for clients, healthcare providers, and care teams to facilitate smooth communication and service delivery.
Schedule appointments, follow-ups, and coordinate care plans in accordance with client needs and healthcare protocols.
Maintain accurate and up-to-date client records.
Assist with administrative tasks such as data entry, filing, and managing correspondence related to care services.
Collaborate with multidisciplinary teams to ensure all aspects of client care are coordinated effectively.
Provide support to clients by answering queries, offering guidance, and ensuring their needs are addressed promptly.
Monitor service delivery progress and escalate issues when necessary to ensure high standards of care are maintained.
Qualifications
The ideal candidate should hold a Level 5 Diploma or NVQ Level 3 qualification (or equivalent) in Health and Social Care or another health-related field, with several years of hands-on experience.
Proven experience in an office environment or administrative role is highly desirable.
Excellent organisational skills with the ability to manage multiple priorities efficiently.
Strong communication skills, both written and verbal, with a compassionate approach towards clients.
Proficiency in office software applications such as Microsoft Office or similar programmes.
Ability to work independently as well as part of a team in a fast-paced environment.
Attention to detail and accuracy in record keeping and documentation.
A genuine interest in healthcare or social services is advantageous but not essential.
