Job Summary


We are seeking an organised, compassionate, and proactive

Care Coordinator

to join our domiciliary care team. The successful candidate will play a vital role in ensuring high-quality, person-centred care is delivered to service users in their own homes. You will be responsible for scheduling care visits, managing care workers, supporting with care planning, and maintaining effective communication between clients, families, staff, and other professionals.

Duties



Care Scheduling:


Efficiently coordinate and allocate care staff to meet client needs, ensuring continuity and punctuality of care.

Client Support:


Conduct initial assessments, contribute to the development of care plans, and regularly review care packages to ensure they remain appropriate and person-centred.

Staff Supervision:


Support, supervise, and mentor care workers, ensuring they are fully briefed on client requirements and have access to the necessary information and resources.

Communication:


Act as the first point of contact for service users, families, and external professionals regarding care delivery matters.

Compliance & Quality Assurance:


Ensure all care provision complies with current legislation, CIW standards, and internal policies and procedures.

On-Call Duties:


Participate in the out-of-hours on-call rota to handle emergencies and ensure continuous care coverage.

Requirements



Essential



Experience in a domiciliary/home care environment Strong administrative, planning, and time-management skills Excellent communication and interpersonal abilities Proficiency with scheduling software and Microsoft Office Understanding of safeguarding, GDPR, and health and safety regulations Ability to work under pressure and in a fast-paced environment Full UK driving licence and access to own vehicle

Desirable:



NVQ Level 3 in Health and Social Care (or working towards) Previous experience in a similar care coordinator role Knowledge of electronic rostering systems (e.g., CM2000, CarePlanner, PeoplePlanner)

Pay: 24,420.00-28,500.00 per year
