Duties



Coordinate care services for clients by liaising with healthcare professionals and service providers. Maintain accurate records of client information, care plans, and progress notes. Schedule appointments and follow-up visits for clients, ensuring timely access to necessary services. Communicate effectively with clients and their families to provide updates on care plans and address any concerns. Assist in the development and implementation of individualised care plans tailored to each client's needs. Monitor client progress and adjust care plans as necessary in collaboration with the healthcare team. Ensure compliance with relevant regulations and standards within the care sector.

Skills



Previous office experience is essential for managing administrative tasks efficiently. Strong communication skills, both verbal and written, to interact effectively with clients, families, and professionals. Excellent organisational abilities to manage multiple tasks and prioritise workload effectively. Proficiency in using office software applications for documentation and scheduling purposes. A compassionate approach towards client care, demonstrating empathy and understanding of their needs. Ability to work collaboratively within a team environment while also being self-motivated.
