Care Coordinator (Part-Time, Hybrid Option)

Location: Portsmouth and surrounding areas

Hours: 3 days per week (flexible)

We're looking for a dedicated and compassionate Care Coordinator to join our small, values-led team. You'll be at the heart of our care operations -- coordinating services, supporting families, and ensuring every client receives thoughtful, high-quality support.

What You'll Do

? Coordinate personalised care plans, ensuring services are responsive and well-matched

? Liaise with healthcare professionals, families, and clients to promote clear communication and shared understanding

? Maintain accurate, compliant client records and documentation

? Schedule appointments and follow-ups, supporting continuity of care

? Contribute to the development of care policies and procedures

? Provide administrative support across the team, including correspondence and reporting

? Monitor client progress and adapt care plans to meet changing needs

? Occasionally cover care calls when needed, supporting continuity and trust
