We are looking for a Care Coordinator who will relish the opportunity to work hard and help reshape the future of our business.
Responsibilities:
Rostering using Access People Planner
Actively managing and maintaining rotas daily
Build a close relationship with senior carers.
Working in partnership with Carers and service users to maintain person centered care scheduling.
Building and maintaining professional relationships with Service users, families and other stakeholders
Maintaining accurate and up-to-date communication records
Managing and organising staff annual leave and recruitment processes
Take part in the on-call rota.
To provide hands on care in the community
Requirements:
Exceptional people management and communication skills
. A good sense of humor
Computer literate (Office 365, excel, word)
Extremely organised and committed
At least 6 months of Care Coordinator experience in a domiciliary care or similar environment
At least 3 years hands on Domiciliary care experience
Achieved or willing to work towards Diploma/ NVQ Level 3 in Health & Social Care
Full UK Driving licence with use of own car.
Demonstrate good leadership qualities.
. A flexible approach to working hours.
Full training will be provided on the Care Management and Rostering systems used in the office as well as all mandatory training.
