Job Description

The Care Coordinator will manage the scheduling and rostering of care services for clients and coordinate with carers to ensure high-quality support. This role plays a key part in balancing the needs of clients with the availability of carers, ensuring that all shifts are covered, and that the service is delivered efficiently.

Key Responsibilities

:

Rostering and Scheduling

: Develop and maintain schedules for carers, ensuring appropriate matching of carers to clients based on needs, preferences, and availability. Adjust rosters promptly to cover shifts due to sickness, holidays, or changes in client requirements. Ensure that shifts are distributed fairly and that carers' hours are compliant with legal and company guidelines. Monitor and update rostering software or tools to track care delivery and staff availability in real-time.

Client Coordination

: Act as the first point of contact for clients regarding their care schedules. Understand client care plans and requirements to match them with the appropriate carers. Respond promptly to changes in client needs, such as urgent changes in care plans, and update rosters accordingly. Ensure clients are informed of any changes to their care schedule in a timely manner.

Carer Coordination

: Communicate with carers regarding their work schedules, responsibilities, and any updates to their rosters. Handle concerns or issues raised by carers related to their schedules and workload. Provide support to carers to ensure they are aware of client needs, care plans, and any specific requirements for each client. Monitor carer availability, ensuring adequate staffing levels are maintained at all times.

Administrative and Reporting

: Maintain accurate records of all care schedules, ensuring that all changes are logged correctly. Prepare and submit regular reports on care delivery, attendance, and other relevant metrics. Ensure compliance with all relevant health and safety regulations and policies. Assist with payroll by ensuring accurate tracking of carer hours worked, including any overtime.

Problem-Solving and Conflict Resolution

: Resolve any scheduling conflicts or last-minute cancellations by ensuring appropriate cover is arranged. Address any issues that arise between clients and carers or among team members, ensuring that high standards of care are maintained. Liaise with other departments as needed to provide holistic client care.

Communication and Collaboration

: Work closely with the care team, management, and other stakeholders to ensure smooth operations. Provide regular updates to clients, carers, and management regarding any changes or issues that arise.

Key Skills and Qualifications

:

Experience

: Proven experience in a care coordination role, preferably with a focus on rostering and scheduling.

Software Proficiency

: Familiarity with rostering software and tools (e.g., CarePlanner, Staffplan, or other care scheduling systems).

Communication Skills

: Excellent verbal and written communication skills for interacting with both clients and carers.

Organizational Skills

: Strong organizational abilities with the capacity to manage multiple schedules and prioritize tasks.

Problem-Solving

: Ability to think quickly and solve issues related to scheduling, carer availability, and client needs.

Interpersonal Skills

: Ability to build good relationships with clients, carers, and team members to foster a supportive working environment.

Education & Experience

:

Diploma or certification in health and social care, business administration, or a related field (preferred). Experience working in a similar care coordination or scheduling role. Knowledge of care standards and regulations is an asset.

Work Hours

:

Full-time (40 hours/week), Monday to Friday. Some weekend or evening availability may be required to handle urgent scheduling changes.
