Job Description

Pulborough Medical Group is looking for further Care Coordinator to join our busy team. This position supports both the clinical and administration teams coordinating care for our 13,500 patients.

We are hoping to recruit someone with NHS experience, especially if it this from previously working in a GP surgery. We would like to employ someone to work between 25-30 hours over 3 or 4 days.

Main Responsibilities include:



Contacting Patients



Working from a task list generated by healthcare professionals and admin teams. This will involve booking in follow up appointments, clinical reviews and giving out results. Completing these tasks by contacting patents via phone call, text message or letter.


eConsults



To be responsible for reviewing all incoming e consults submitted by patients on a daily basis during working hours. To liaise with clinicians as appropriate and book appointments for patients as required.


Waiting lists



To be responsible for monitoring the patient waiting lists.

To contact and book patients in for procedures.

To advise Practice Manager when clinics are required and monitor uptake of these clinics.


Covid and Flu Vaccines



To be a point of contact for patients and assist the practice manager with the recalling of these patients for each phase of the vaccination programme.


Pathology Results



To help reduce GP clinical workload care coordinators will assist doctors with the filing of high volume, low clinical risk pathology tests using evidence-based decision aids (Path Go).


Friends and Family Test



Collecting, optimising and analysing responses from patients on a monthly basis, and reporting the finding to the practice manager.


DMARDs (Disease-modifying Anti-rheumatic Drugs)



Maintaining the drug monitoring register and checking shared-care agreements are in place,

arranging appointments and

c

arrying out audits as required.


Quality Outcomes Framework (QOF)



To contact patients in a timely manner for their QOF reviews.

To ensure all reviews are organised when contacting the patient.

To work through and complete tasks sent by Project Manager.

To use the Systmone generated reports for each of the QOF domains to contact patients.

To use a variety of methods to contact patients including phone, text or letter.

To code all contacts accurately on the patients' clinical record.

To set up systems to follow up patients, who could not be contacted, to ensure their reviews are organised.

To monitor progress on QOF and assist the Project Manager so that our patients on the QOF disease registers get the best care.


Blood Pressure Monitoring



Acting on Health Kiosk alerts

Processing of 7 day BP diaries

Actioning Hypertension QOF indicators


Learning Disability and Mental Health Patients



Being point of contact for patients on LD register and Mental Health register

Validating the LD register

Ensuring annual reviews are booked in a timely manner


Under 16 Did Not Attend



Following up any appointment DNA for children


Assistance with general administrative tasks including:



Assisting the call centre if cover required

Appointment booking

Vaccination schedule monitoring and planning

Processing prescription requests

Attendance at meetings and recording of minutes

Gathering statistical information as required
