This position involves coordinating care services, managing and supporting support workers, liaising with healthcare professionals, maintaining accurate records, and covering any shortfalls in staff rotas. The Care Coordinator will work closely with clients, their families, and various professionals to ensure that all needs are met efficiently and compassionately.
Key Responsibilities
Coordinate and manage care plans for clients, ensuring all services are delivered on time
Serve as the primary point of contact for clients, families, and professionals, providing support and guidance
Liaise with professionals to facilitate comprehensive care
Maintain accurate and up-to-date records of client information, care plans, and service delivery
Conduct regular follow-ups with clients to assess their needs and satisfaction
Assist in the development of new programmes or services based on client feedback and emerging needs
Ensure compliance with all relevant regulations and standards within the healthcare sector
Participate in meetings and contribute to continuous improvement of care services
Qualifications & Skills
Previous experience in a childcare service, health, or social services setting is preferred
Strong organisational skills with the ability to manage multiple tasks effectively
Excellent verbal and written communication skills for interacting with clients, families, and professionals
Ability to help build and maintain client lists
Proficiency in using office software for documentation and communication
A compassionate approach to client care and a strong commitment to enhancing client well-being
Ability to work independently as well as part of a collaborative team
Relevant qualifications in health or social care are advantageous but not essential
Cover shortfalls in staff rotas as required to maintain seamless care delivery
