Indeed Title: Care Coordinator (Domiciliary Care)



Overview



We are looking for a dedicated and highly organised Care Coordinator to join our team. In this pivotal role, you will ensure that service users receive seamless and personalised care by coordinating their support services, managing schedules and liaising with staff, service users and families. The ideal candidate will have excellent IT skills, strong communication abilities and the capacity to manage a varied workload with professionalism and attention to detail.

Duties & Responsibilities



Coordinate care plans for service users, ensuring they are up to date and meet individual needs. Create, manage and adjust Rotas that align with client preferences and staff availability. Supervise and support care staff, offering guidance, mentoring and oversight. Undertake initial assessments / meet?and?greets with new service users to determine suitability. Monitor service user satisfaction; follow up to ensure they are content with the care provided. Produce meaningful reports for the Nominated Individual and Directors, including data on outcomes, incidents, staffing and feedback. Use Microsoft Word, Excel and care software to maintain accurate records, communicate with stakeholders and analyse data. Ensure compliance with CQC regulations, company policies, safeguarding requirements and health & safety standards. Manage office administrative tasks, including handling referrals, invoicing coordination and ensuring document accuracy. Work alongside senior management to ensure business goals are met and new referrals are handled smoothly.

Skills & Qualifications



Experience in domiciliary care coordination or in a supervisory role in health/social care. Excellent written and verbal communication skills; professional in email correspondence and reporting. Strong IT proficiency, especially MS Word, Excel and care management systems. Highly organised, able to prioritise tasks, meet deadlines, and manage a busy workload. Strong interpersonal skills; ability to engage service users, staff, and other professionals. Supervisory or leadership experience with ability to support and manage staff. Attention to detail and problem-solving skills; ability to anticipate and resolve issues. Ability to travel / access to car and valid UK driving licence (if role involves field/staff visits). Commitment to person?centred care and quality, including following care plans and ensuring continuity of service.
