We are seeking a dedicated and organised Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care and support. As a Care Coordinator, you will be responsible for managing client care plans, liaising with healthcare professionals, and providing exceptional service to clients and their families. This position requires strong communication skills and the ability to work effectively in an office environment.

Duties

Develop and maintain individual care plans for clients, ensuring they meet their specific needs. Coordinate with healthcare providers, social workers, and other professionals to facilitate comprehensive care. Monitor client progress and adjust care plans as necessary, maintaining accurate records. Serve as the primary point of contact for clients and their families, addressing any concerns or questions. Organise appointments, follow-ups, and transportation arrangements for clients as needed. Maintain confidentiality and adhere to all regulatory requirements related to client information. Participate in team meetings to discuss client progress and share insights on care improvements.

Qualifications

Previous experience in a healthcare or social services setting is preferred. Strong organisational skills with the ability to manage multiple tasks simultaneously. Excellent communication skills, both verbal and written, with a compassionate approach. Proficiency in using office software applications (e.g., Microsoft Office Suite). Ability to work independently as well as part of a team. A commitment to providing high-quality care and support to clients.
