We are a leading provider of domiciliary care services, dedicated to delivering high-quality care and support to individuals in their own homes. We are seeking an experienced and compassionate Domiciliary Care Coordinator to join our team.
Job Summary:
As a Domiciliary Care Coordinator, you will be responsible for coordinating care services for our clients, ensuring that their needs are met and that they receive the best possible care. You will work closely with our care team, clients, and their families to develop and implement personalized care plans.
Key Responsibilities:
- Coordinate care services for clients, including scheduling visits and managing care rotas
- Assess client needs and develop personalized care plans
- Work closely with care staff, clients, and their families to ensure that care plans are implemented and reviewed regularly
- Monitor and report on client care and well-being
- Maintain accurate records and reports
- Communicate effectively with clients, families, and healthcare professionals
Requirements:
- Experience in a care coordination or social care role
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and person-centred approach to care
- Ability to work in a fast-paced environment
-Managing customer visits and arranging reviews and quality checks including spot checks and supervision
- Basic computer skills and ability to use care management software
-In emergency situations care- coordinators will need to cover service user calls to ensure continuity of care
