Bridge to Care Housing Ltd is a nurse-led supported living provider based in Sheffield. We support adults with Mental health, learning disabilities, autism, and complex needs to live independently and thrive within their community. We are now looking for a Care Coordinator to join our growing team.

About the Role:



As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality, person-centred care. You'll coordinate care packages, support staff development, and work collaboratively with professionals, families, and service users to ensure each person's needs are met with dignity and respect.

Responsibilities:



Develop and review personalised care and support plans Conduct risk assessments and contribute to multi-agency reviews Support and supervise support workers, including performance monitoring and mentoring Ensure service delivery meets CQC standards and internal quality benchmarks Liaise with external professionals (social workers, GPs, CMHTs) Maintain accurate digital records and rota planning Promote independence, inclusion, and well-being for every individual

Requirements:



Level 3 Diploma in Health and Social Care (or equivalent) - essential Minimum 2 years' experience in adult care, with at least 1 year in a supervisory or coordination role Strong understanding of safeguarding, person-centred care, and risk assessment Excellent communication, leadership, and organisational skills Confident with care planning systems and IT Flexible to work occasional evenings/weekends if required Previous experience in a healthcare or social services setting is preferred. Strong organisational skills with the ability to manage multiple tasks simultaneously. Excellent verbal and written communication skills. Proficient in using office software applications (e.g., Microsoft Office Suite). Ability to work independently as well as part of a team. A compassionate approach with a genuine desire to help others.

Desirable:



Experience in supported living, Domiciliary Care or complex care Level 5 Diploma (or willingness to work towards) Driving licence and access to a vehicle
