The Role


The Care Coordinator plays a pivotal role in ensuring that clients receive comprehensive and well-coordinated care. This trusted position involves liaising between clients, family members, health professionals, our Managers, and our team of Angels who provide care in the field.

Strong and effective communication skills are essential, along with a professional office manner and a light-hearted, family-oriented attitude that reflects our friendly values.

This is a

full-time, office-based role

in our new Shrewsbury office at Shrewsbury Business Park, Monday to Friday, 09:00-17:00. The role also includes

one evening and one weekend in every 5 or 6

as the on-call person, with

an additional 100 per weekend

.

Duties



Coordinate rosters in a timely manner, ensuring shadowing and introductions are arranged and Angels are supported. Ensure clients and Angels have a positive experience throughout their care journey. Onboard clients in line with company policies and expectations. Foster good relationships between Caregiver Angels, clients, and the office team. Maintain accurate notes and records. Collaborate with healthcare professionals and family members to develop tailored care plans. Raise and escalate any safeguarding concerns to the Registered Manager. Provide excellent communication and support to anyone contacting the office, whether face-to-face, by phone, or email. Take part in the on-call rota (currently one night per week and one weekend in 5 or 6), handling calls, queries, and any absence/sickness to maintain service standards. Additional pay: 100 per weekend. Must be able to drive and cover care calls when required.

Experience



Previous experience in a similar role or in care is advantageous. Experience using the Birdie system (or another rota system) is desirable. QCF Level 3 or above in Health and Social Care is advantageous. Strong organisational skills with the ability to manage multiple tasks in a busy office environment, with great attention to detail. Excellent verbal and written communication skills for interacting with clients and healthcare professionals. Proficiency in MS Office (Excel and Outlook). Confidence using social media platforms such as LinkedIn, Facebook, Instagram, and TikTok. A compassionate approach towards our Angel Care Team and Clients, with a genuine desire to help.
