Walfinch Windsor and Maidenhead is seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that our clients receive the highest quality of care and support. Responsibilities
Serve as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
Coordinate care plans by collaborating with healthcare professionals, social workers, and other relevant parties.
Maintain accurate records of client information, appointments, and care plans within the office environment.
Schedule appointments and follow-up visits, ensuring that clients receive timely services.
Monitor client progress and adjust care plans as necessary to meet evolving needs.
Provide support to clients in navigating healthcare systems and accessing resources.
Conduct regular check-ins with clients to assess satisfaction with services provided.
Assist in the development of educational materials for clients and families regarding available services.
Qualifications
Previous experience in a healthcare or social services setting is preferred but not essential.
Strong organisational skills with the ability to manage multiple tasks effectively within an office environment.
Excellent communication skills, both verbal and written, with a focus on empathy and active listening.
Proficiency in using office software applications for documentation and communication purposes.
Ability to work collaboratively as part of a team while also being self-motivated.
