Job Description

Coastal PCN is looking for a full-time Care Co-ordinator to join our growing team. The role is mainly based within the surgeries and the post holder may need work within more than one practice, so the ability to be flexible and willing to work alongside other colleagues from all four member practices is essential.

We believe this is an exciting opportunity to help deliver integrated care based around the needs of our practice population. We are looking to appoint patient facing individuals who will play a key role at the heart of our GP Practices.

Our Primary Care Network (PCN) has a patient population of over 29,000 people set in beautiful mid Cornwall, incorporating 4 local GP practices (St. Agnes, Perranporth, Carnon Downs & Chacewater), which have excellent inter-relations and a long history of working positively together. Our population is diverse compared to other areas in Cornwall, with varying levels of socio-economic status, young families and elderly patients all of whom make providing care interesting and rewarding.

We are highly passionate about providing good quality patient care and all four practices regularly appear in the top ten practices in Cornwall for patient satisfaction.

Job summary



The Care Coordinator will ensure our patients feel supported and have access to the services that are right for them, helping them to manage their own needs where possible, promote and encourage patient awareness, skills and confidence and to help patients maintain and make decisions about their own health and wellbeing. There will be a focus on those patients who are harder to reach or may have access or unmet needs.

Job responsibilities



Supporting patients on an ongoing basis to manage their own needs where possible, promote and encourage patient awareness, skills and confidence to them help maintain, and make decisions about their own health and wellbeing, including personal health budgets Help patients to manage their care needs through answering queries, making and managing appointments and ensuring that patients have good quality written or verbal information relating to their care and medication. Arrange appointments at the practices for patients directed by clinicians following identification of urgent and non-urgent clinical need to assess diagnose and treat according to the patients health needs. Refer patients through to the appropriate member of the team, and/or make referrals on behalf of the team to external support or care groups as appropriate. Signpost patients to appropriate community or local services. Serve as the contact point, advocate and informational resource for patients, care teams, family /caregivers and community resources where indicated, responding with empathy and respect. Support the PCNs Practices to deliver on quality metrics such as QOF, KPIs and locally commissioned enhanced services (full training provided). Maintaining excellent communication levels with patients to check on the care they are receiving and keeping relevant clear documentation Contribute to the development, and support implementation, of patient management plans and associated innovative services and projects. Work closely with colleagues as part of the PCN Frailty team, supporting housebound patients with the appropriate ongoing care within their own homes.
