Overview


The Care Coordinator plays a pivotal role in ensuring that patients receive the highest quality of care throughout their healthcare journey. This position involves coordinating various aspects of patient care, facilitating communication between patients and healthcare providers, and ensuring that all necessary services are delivered efficiently and effectively. The ideal candidate will possess strong organisational skills and a compassionate approach to patient care.

Duties



Serve as the primary point of contact for patients, addressing their questions and concerns regarding their care. Coordinate appointments, referrals, and follow-up services to ensure seamless patient experiences. Maintain accurate and up-to-date patient records, ensuring compliance with all relevant regulations. Collaborate with healthcare professionals to develop and implement individualised care plans tailored to each patient's needs. Facilitate communication between patients, families, and medical staff to promote understanding and adherence to treatment plans. Monitor patient progress and provide support as needed, including arranging additional resources or services. Assist in administrative tasks within the office environment to ensure smooth operations.

Strong organisational abilities with a keen attention to detail. Excellent communication skills, both verbal and written, to effectively interact with patients and healthcare professionals. Proficiency in office software applications for maintaining records and coordinating schedules. Empathy and compassion towards patients, demonstrating a commitment to providing high-quality care. Ability to work collaboratively within a team environment while also being self-motivated. Problem-solving skills to address any challenges that may arise in patient care coordination.

Pay: From 13.85 per hour
