Job Title: Experienced Care Coordinator (Full-Time)
Job Summary:
We are seeking an experienced and compassionate Care Coordinator to join our team at Topioneer Care Ltd. As a Care Coordinator, you will be responsible for assessing the needs of our clients, developing personalized care plans, and coordinating care services to ensure our clients receive the support they need to live independently in their own homes.
Key Responsibilities:
1. Assess Client Needs: Conduct thorough assessments of clients' needs, abilities, and preferences to develop personalized care plans.
2. Develop Care Plans: Create and implement care plans that meet the unique needs of each client, ensuring their physical, emotional, and social well-being.
3. Coordinate Care Services: Arrange for care services to be delivered by our team of caregivers, ensuring that clients receive the support they need in a timely and efficient manner.
4. Monitor and Review Care Plans: Regularly review and update care plans to ensure they remain relevant and effective, making changes as needed.
5. Communicate with Clients and Families: Build strong relationships with clients and their families, keeping them informed about care plans, services, and any changes.
6. Collaborate with Healthcare Professionals: Work closely with healthcare professionals, such as doctors, nurses, and social workers, to ensure seamless care transitions and comprehensive support.
7. Maintain Accurate Records: Keep accurate and up-to-date records of client assessments, care plans, and service delivery.
8. Risk Management: Identify potential risks and develop strategies to mitigate them, ensuring the safety and well-being of our clients.
9. Caregiver Management: Supervise and support caregivers, providing guidance and feedback to ensure high-quality care delivery.
10. Quality Improvement: Participate in quality improvement initiatives, contributing to the development of policies and procedures that enhance our care services.
Requirements:
- Experience: Minimum 2-3 years of experience in a care coordination role, preferably in home care or a similar setting.
- Track Record: A proven track record of successfully coordinating care services, improving client outcomes, and working effectively with clients, families, and healthcare professionals.
- Qualifications: Relevant qualifications in social care, nursing, or a related field (e.g., NVQ Level 3 in Health and Social Care, Diploma in Social Care).
- Skills:
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Compassionate and empathetic approach to care coordination.

Working Hours:
- Full-Time position, working 37.5 hours per week
- Monday to Friday, 9am-5pm (with a 30-minute lunch break)
