Main duties of the job




The Care Coordinator within an acute care ward will be responsible for coordinating aspects of patient care including liaison with other agencies both internal and external to CWP.



The post holder will be the central coordination point forward based multi-disciplinary activity, organising and supporting with ongoing regular meetings to facilitate progress to recovery and discharge with relevant CWP team members, relatives and carers and other external agencies.

Essential criteria




Good general education Able to demonstrate a good command of English language, verbal and written. Knowledge of Office Administration/Custo mer service acquired through training or experience to NVQ level 3 or equivalent #

Knowledge and Expertise

Knowledge of Health and Social Care administration and processes. Knowledge of Microsoft office applications, internet and email

Knowledge/familiarity with medical terminology #

Able to work well in a team. Experience of working in an Office Environment. Experience/knowledge of issues of working with confidential information Experience of working with healthcare professionals/previous experience in the NHS.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
