The Care Coordinator is responsible for supporting the safe and effective delivery of
adult home care services
. This role involves completing client assessments, supporting the onboarding of new care packages, conducting spot checks on care staff, and maintaining high standards of care delivery.
Previous experience in complex care is
not required
, as full training will be provided. However, a
full UK driving licence and access to a vehicle is essential
, as the role involves travelling to clients' homes to complete assessments, new package setups, and quality monitoring visits.
Key Responsibilities
Complete initial and ongoing
care assessments
for adults receiving care in their own homes
Support the setup and onboarding of
new care packages
, ensuring documentation is completed accurately
Assist with the development and review of care plans and risk assessments
Carry out
regular spot checks
and unannounced visits to care staff working in the community
Monitor care delivery to ensure safe practice, professionalism, and compliance with care plans
Provide feedback and guidance to care staff following spot checks
Act as a point of contact for care staff, clients, and families during working days
Escalate concerns relating to care quality, safeguarding, or staff performance to management
Review care documentation including daily notes, MAR charts, and incident reports
Support compliance with organisational policies, procedures, and CQC standards
Maintain accurate records and ensure confidentiality at all times

Essential Requirements
Strong organisational and communication skills
Ability to work independently and manage a varied workload
Professional and confident approach when dealing with clients and staff
Full UK driving licence and access to a vehicle - essential
Willingness to learn and undertake training relevant to the role

Previous experience in health or social care
Experience completing assessments or quality monitoring visits
Knowledge of adult home care services
NVQ Level 2 or 3 in Health and Social Care (or willingness to work towards)
