Care Coordinator



Location: GREENWICH, WOOLWICH-LONDON

BCC is a Home Care (support services), Domiciliary, and Supported Living organisation based in Greenwich/Woolwich, providing services across South East London and other boroughs.

We support vulnerable adults within the community who require assistance with daily living due to conditions such as mental health needs, autism, learning disabilities, and elderly care requirements.

Essential Criteria:



Applicants must possess an

NVQ Level 3 or Level 4 qualification (or equivalent)

.

A minimum of

one year and six months' proven experience

in

domiciliary care and support services

is required.

Applicants must demonstrate

appropriate skills and knowledge in care

, along with a willingness to undertake further training and professional development.

We are seeking a

Care Coordinator

who exhibits

excellent communication and organisational skills

and consistently demonstrates

flexibility, commitment, and reliability

.

The successful candidate will be responsible for coordinating and supporting

care delivery within London and the surrounding areas



Care Coordinator - Key Responsibilities:



Work collaboratively as part of the team, reporting directly to the

Registered Manager

.

Flexibility, Commitment, and Reliability

.

Promote and uphold

person-centred care

, supporting and guiding staff to meet the assessed needs of

Service Users

.

Maintain

regular and effective communication

with carers and Service Users to ensure continuity and quality of care.

Ensure the

delivery of high-quality care and support

in line with organisational policies and regulatory standards.

Participate in the

on-call rota

, providing operational support when required.

Develop, implement, and review

care plans

in accordance with individual needs and risk assessments.

Prepare and manage

staff rotas

using the organisation's

digital scheduling system

.

Conduct and complete

audits

to monitor compliance, quality assurance, and service improvement.

Liaise effectively with

external multidisciplinary teams (MDTs)

involved in the care of Service Users.

Proactively identify and seize opportunities to support other areas of the business aligned with your skills and experience, including

assessments

and

representation of the organisation at external meetings or events

.

Candidate Requirements (Qualifications and Competencies):



Demonstrable

administrative and IT skills

, with the ability to use digital systems effectively.

Sound

knowledge of legal responsibilities and regulatory requirements

within health and social care.

Proven understanding of

person-centred care

, including the development of

care plans

,

risk assessments

, and

initial assessments

.

Excellent

organisational and communication skills

, with the ability to manage multiple priorities effectively.

Candidates must be able to

prove their right to work in the UK

, provide

satisfactory references

, and successfully complete an

enhanced DBS check

.

full, valid UK driving licence

and be able to provide evidence thereof, due to the operational requirements of the role.
