Langley House GP Surgery has a vacancy for a full-time role for a care co-ordinator/administrator.

Previous experience working in the NHS or a health care provider and working in primary care would be an advantage.
Key role requirements
Care coordinators will:
Provide support for our care homes and frail patients, including registration of new care home patients.
Provide support for our care home by processing repeat prescriptions
Organise a programme of care planning and medication reviews by clinicians
Assist the practice in the management and recall of patients with long-term conditions so that the practice can achieve key health targets.
Data entry of clinical information into patient records
Assist our clinical team by conducting basic diagnostic procedures
Be responsible for stock control in consulting rooms
Key areas of Knowledge
Strong administrative skills and to be proficient at using multiple IT tools to review and update patient data including Word and Excel.
Strong communication skills - effectively communicate at all levels of the organisation to a variety of health professionals, patients and carers to provide the best outcomes.
A knowledge and understanding of Health and Social Care terminology and familiarity with symptoms of medical conditions would be beneficial but training can be given
Full-time role 9 am - 5 pm Monday-Friday

Pay: 13.00-13.81 per hour
Expected hours: 35 per week
