Job Description

Job Overview We are seeking a dedicated and compassionate Care Coordinator / Admin to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the highest quality of care by facilitating communication between patients, families, and healthcare providers. This position requires strong organisational skills and the ability to manage multiple tasks efficiently in an office environment.

To also support the management team with recruitment, field work to include covering care calls when needed and participating in the on call rotas which is an additional pay!

Completing assessments and reviews of clients in a timely and professional manner

You will also be required to support our second office in Oxford twice weekly days and times to be discussed.

We are flexible on times this can be discussed on interview

Responsibilities- Serve as the primary point of contact for patients and their families, addressing inquiries and concerns with empathy and professionalism. - Coordinate patient care by scheduling appointments, follow-ups, and necessary referrals to specialists. - Maintain accurate and up-to-date patient records, ensuring compliance with confidentiality regulations. - Collaborate with healthcare professionals to develop and implement individualised care plans tailored to each patient's needs.
- Monitor patient progress and communicate updates to relevant stakeholders, including family members and healthcare teams. - Assist in the management of office operations, including maintaining supplies and managing correspondence. - Provide support in the development of educational materials for patients regarding their care options and resources available.

Qualifications - Previous experience in a healthcare or office setting is preferred but not essential. - Strong organisational skills with an ability to prioritise tasks effectively in a fast-paced environment. - Excellent communication skills, both verbal and written, with a focus on providing exceptional customer service. - Proficiency in using office software applications (e.g., Microsoft Office Suite) is desirable. -

Pay: 12.85-13.00 per hour
