Care Co-ordinator Pharmacy Team Assistant Location: Lionwood Medical Practice, 30a Wellesley Avenue North, Norwich, NR1 4NU Website: lionwoodmedicalpractice.nhs.uk Job Summary We are looking for a friendly and motivated care co-ordinator with a specific interest in medication management to join our supportive GP practice team. This role combines the responsibilities of a Care Co-ordinator and a Pharmacy Assistant, focusing on delivering personalised, patient-centred care and supporting the pharmacy team in medication-related services. The postholder will work closely with clinical and non-clinical staff to ensure patientsparticularly those with complex needsreceive coordinated care and appropriate medication support. This is a patient-facing role, designed to support both the pharmacy team within the GP practice and the wider local community.



Main Duties of the job Help patients understand their medications, including dosage and side effects. Complete basic medication reviews and manage patients long term conditions under pharmacist supervision. Deliver patient-facing and supporting roles to ensure safe, effective medicines use through shared decision-making. Undertake medicines optimisation tasks such as checking inhaler technique, and supporting medicines reconciliation Perform administrative duties related to pharmacy operations.



Support in the supervision of reception and prescription teams in sorting prescription requests and medication related queries Provide specialist expertise (where competent) on public health and social care needs, including lifestyle advice, service information, and tackling health inequalities. Creating and maintaining positive, close relationships with local pharmacies to improve team-working and patient care Assist with purchasing, receiving, and managing GP stock of medicines. Facilitate and monitor personalised care plans, ensuring timely reviews and updates. Liaise between patients, clinical staff, and external services to streamline care pathways.



Support patients and carers in accessing appropriate health and social care services. Maintain accurate patient records and contribute to clinical reporting systems. Collect and interpret patient data for service evaluation and improvement. Work closely with care agencies to ensure coordinated care without requiring further GP referral.



Job Responsibilities This role supports the delivery of high-quality, personalised care and safe medication management for patients registered at Lionwood Medical Practice. The postholder will work across clinical and non-clinical teams to ensure patientsparticularly those with long-term conditions or complex needsreceive coordinated support both medically and socially. Person Specification Qualifications Essential: Evidence of consistent patterns of learning from education, training, and experience Desirable: Qualification in health or social care allied profession or Health Care Assistant Role Completion of Pharmacy Medicines counter assistance course or equivalent Dispensing Assistant Course accredited by the General Pharmaceutical Council. Experience Essential: Strong communication and relationship-building skills.



Experience of developing relationships with a wide variety of people Desirable: Experience in pharmacy or medicines-related work. Experience in inpatient environments or ward-based settings. Experience in GP or primary care environments Familiarity with NHS pharmacy services and medicines management. Experience with health and social care assessments.



Experience in health or social care Skills and Knowledge Essential: Understanding of human needs physical, emotional, social Basic health and social care terminology. Good organisation, prioritisation, and time management skills. Strong verbal and written communication. Ability to work independently and as part of a team.



Basic IT skills including Microsoft Office. Attention to detail and accuracy. Understanding of confidentiality and safeguarding. Desirable: Knowledge of medicines management and relevant legislation.



Experience of administrative duties Experience in use of databases. Familiarity with IT systems currently in use in health and social care settings e.g systmone Evidence of excellent knowledge of Microsoft Office Attributes Highly organised and reliable. Adaptability and a flexible, positive approach Motivated and able to work with minimal supervision. Sensitive to patient needs and confidentiality.



Willingness to undertake further training. Honest, trustworthy, and able to use initiative appropriately. Able to work under pressure and meet deadlines. Sensitive to patients needs.
