Job role and purpose:



Our Care Lead role is to support the Registered Manager of a mental health rehabilitation service that utilises the Open Dialogue approach and will be responsible for leading the day to day management within the home ensuring high standard of care is provided at all times, implementing the Open Dialogue approach internally and externally and maintaining standard, providing support to individuals with mental health needs.

The Care Lead will be required to lead a team of support workers, ensuring the delivery of personalized care, and promoting the well-being and independence of residents. Care Lead will act as a key point of contact for residents, their families, and external professionals, fostering a positive and inclusive environment.

Main duties and responsibilities:



The main duties and responsibilities of the role are:

Care and Support



Provide high-quality personal care and support to residents, including assistance with daily living activities, personal hygiene, and mobility.

Develop and implement individualized care plans in collaboration with residents, families, and healthcare professionals.

Monitor and evaluate the effectiveness of care plans and interventions, making recommendations and suggestions as needed to ensure positive outcomes for the people we care for.

Monitor and report on residents' physical and emotional well-being, ensuring any changes are addressed promptly.

Support individual care plans in partnership with the people we care for, their families and friends and other professionals involved in their care.

Foster a culture of inclusivity, respect, and empowerment within the service, promoting the rights and dignity of service users.

Provide medication support as required after undergoing appropriate training

Team Leadership



Provide leadership and guidance to care staff, promoting a positive working environment and ensuring high standards of care and professionalism

Lead and manage a team of staff, including support workers and Experts by Experience, to provide person-centred care and support. Thus, leading the shift and delivering care accordingly.

Supervise and mentor junior support workers, providing guidance, training, and support to ensure they deliver effective care.

Foster a positive team environment, promoting open communication and collaboration among staff members.

Support the induction and supervision of staff, promoting a culture of continuous learning and professional development.

Assist in staff training and orientation, ensuring all team members understand their roles and responsibilities.

Health and Safety



Ensure that all health and safety policies and procedures are followed, maintaining a safe environment for residents and staff.

Adhere to the policies of the organisation and supporting care staff to do this also to ensure quality assurance of the care we provide.

Administer medications as prescribed and ensure accurate record-keeping in compliance with care regulations.

Respond to emergencies and incidents, providing appropriate support and intervention as needed.

Documentation and Reporting



Maintain accurate and up-to-date records of residents' care, daily activities, and incidents in accordance with regulatory requirements.

Participate in regular reviews of care plans and contribute to the evaluation of care delivery and outcomes.

Prepare reports and documentation for team meetings and reviews.

Support the implementation of the Open Dialogue approach in the delivery of care and treatment to the people we care for, promoting collaborative decision-making and fostering open communication.

Serve as a primary point of contact for residents and their families, addressing concerns and providing information about care services.

Liaise with external healthcare professionals and agencies to coordinate care and support for residents. Promote a culture of inclusion and respect, encouraging residents to participate in decisions about their care.

Contribute to the development and implementation of quality improvement initiatives within the facility.

Engage in ongoing professional development, attending training sessions and workshops to enhance skills and knowledge.

Pay: From 13.50 per hour
