We are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within assisted living and care home environments. The successful candidate will be responsible for managing care plans, supervising staff, and ensuring the wellbeing of residents, particularly those requiring specialised
dementia and D2A (Discharge to Assess)

Duties
Develop, implement, and review personalised care plans tailored to individual needs, including
dementia and D2A care pathways
.
Supervise and lead a team of care staff, ensuring adherence to care standards and regulatory requirements.
Oversee medication administration programmes, ensuring safe and accurate delivery of prescribed treatments.
Coordinate with healthcare professionals to support
D2A transitions
, nursing, and nursing home services.
Monitor residents' health and wellbeing, responding promptly to any changes or concerns.
Organise staff rotas, manage shift coverage, and provide supervision to ensure seamless service delivery.
Lead training sessions on
dementia care, D2A processes
, medication management, and other relevant topics to maintain high standards of care.
Foster a positive environment that promotes independence, dignity, and respect for all residents.
Experience
Proven experience in senior or elder care settings such as assisted living or nursing homes.
Extensive knowledge of
dementia care and D2A models of care
.
Demonstrable management or supervisory experience within a care home or similar environment.
Familiarity with creating and managing personalised care plans.
Strong leadership skills with the ability to motivate and guide a diverse team.
Experience in medication administration and compliance with healthcare regulations.
Knowledge of home care services is desirable but not essential.
