Job Summary

We are seeking a dedicated and compassionate Care Manager to join our team. The ideal candidate will possess a strong background in behavioral health and social work, with expertise in crisis intervention and management. As a Care Manager, you will play a vital role in supporting individuals and families through challenging situations, providing guidance and resources to promote overall well-being. This position requires excellent communication skills, the ability to work collaboratively, and a commitment to empowering clients towards positive outcomes.

Responsibilities

Conduct comprehensive assessments to identify client needs and develop individualized care plans. Provide crisis intervention and management services to clients facing immediate challenges. Offer addiction counseling and support for individuals struggling with substance use disorders. Facilitate access to behavioral health services, including therapy and medication management. Mentor and supervise junior staff members, fostering professional development within the team. Collaborate with community resources, including Section 8 housing assistance and workforce development programs. Advocate for clients within the child welfare system, ensuring their rights and needs are met. Maintain accurate documentation of client interactions, progress notes, and treatment plans in compliance with regulatory standards.

Experience

Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred. Proven experience in crisis intervention, behavioral health services, or addiction counseling. Familiarity with child welfare systems and Medicare regulations is highly desirable. Strong mentoring skills with prior supervising experience preferred. Proficiency in motivational interviewing techniques to engage clients effectively. Experience working within childcare settings or related environments is a plus.
