Overview

We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care in our facility. The ideal candidate will possess a strong background in senior care, with experience in domiciliary care, and dementia care. As a Care Manager, you will be responsible for developing and implementing care plans, managing staff, and ensuring that residents receive the best possible support tailored to their individual needs.

Responsibilities

Develop and implement comprehensive care plans for residents, ensuring they meet individual needs and preferences. Supervise and manage care staff, providing leadership and support to ensure high standards of care are maintained. Monitor the well-being of residents, conducting regular assessments and adjusting care plans as necessary. Facilitate communication between residents, families, and healthcare professionals to ensure cohesive care delivery. Oversee medication administration and ensure compliance with safety regulations. Provide training and guidance to staff on best practices in senior care, including Alzheimer's and dementia care. Maintain accurate records of resident care and staff performance for regulatory compliance. Foster a positive environment that encourages resident engagement and independence.

Requirements

Proven experience in domiciliary care management is essential. Background in nursing or relevant healthcare qualifications is highly desirable. Strong knowledge of senior care practices, including dementia and Alzheimer's care. Experience in developing care plans tailored to individual needs. Demonstrated management skills with experience supervising teams in a healthcare environment. Excellent leadership abilities with strong communication skills to interact effectively with residents, families, and staff. Familiarity with medication administration protocols is required.
