Every GP Surgery needs an efficient team of Care Navigator-Care Coordinators who can ensure that every patient's experience is a positive one. We are looking to recruit an enthusiastic, motivated and outgoing individual to join our friendly team. The position will involve reception, secretarial and administrative duties and you will be part of the front of house team.
Role Summary:
Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way
Undertake a variety of administrative duties to assist in the smooth running of the practice, including the provision of secretarial and clerical support to clinical staff and other members of the practice team
Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies
Respond to changing circumstances with a positive and flexible approach, displaying a calm and courteous manner throughout.
Duties and responsibilities:
These will include, but are not confined to:
Processing personal and telephone requests for appointments, visits and telephone consultations, and ensuring callers are directed to the appropriate healthcare professional
Scan, code and summarise patient records in accordance with practice guidelines
Process referrals, practice correspondence, including emails and other electronic or paper copies
Explain services and systems to patients, e.g. appointments system, repeat prescriptions system, referrals.
Register new patients and process changes of details, ensuring all appropriate checks are made
Provide care coordination services to frail, elderly and housebound patients
Previous experience of coding, summarising and document triage/workflow is preferable, but training will be offered.
GCSEs in Maths and English, at level 2 or above (GCSE grade C equivalent) are essential for this post.
Salary negotiable and dependent on experience.
