The Care Navigator plays a crucial role in assisting clients through their healthcare journey. This position involves coordinating care, providing support, and ensuring that clients have access to the resources they need. The ideal candidate will possess strong organisational skills and a background in administrative tasks, enabling them to manage multiple responsibilities effectively while maintaining a high level of customer service.

Responsibilities



Serve as the primary point of contact for clients, addressing inquiries and providing information about available services. Coordinate appointments and follow-up care, ensuring that clients receive timely support. Maintain accurate records and documentation of client interactions and care plans. Utilise various software applications, including Microsoft Office , EMIS Clinical system, AccuRx and Docman, for data entry and reporting purposes. Assist with administrative tasks such as typing correspondence, managing schedules, and maintaining office supplies. Ensure compliance with organisational policies and procedures while handling sensitive information.

Skills



Proven office experience within a GP surgery with a strong emphasis on administrative functions. Proficient computer skills, including familiarity with EMIS Clinical System, AccuRx, Docman, Microsoft Office Suite (Word, Excel, PowerPoint) applications. Excellent phone etiquette to communicate effectively with clients and healthcare professionals. Strong organisational skills to manage multiple tasks efficiently while prioritising client needs. Accurate typing skills for data entry and documentation purposes. Experience and understanding of Quality & Outcomes Framework -QOF, Quality Framework - QF - Impact and Investment Fund - IIF and the impact it has on General Practice Previous clerical experience is preferred to support the administrative demands of the role. This role is ideal for individuals who are passionate about helping others navigate their healthcare options while utilising their administrative expertise in a supportive environment. Pay: 12.21-14.76 per hour
