The ideal candidate will play a crucial role in supporting clients by guiding them through their care journey, ensuring they receive the necessary services and resources. This position requires strong organisational skills, excellent communication abilities, and proficiency in various administrative tasks. The Care Navigator will work closely with clients, healthcare providers, and community resources to facilitate effective care coordination.
Responsibilities
Assist clients in navigating healthcare services and resources effectively.
Maintain accurate records and documentation of client interactions and care plans.
Perform data entry tasks to ensure all information is up-to-date and organised.
Provide clerical support including handling phone enquiries with professionalism.
Collaborate with healthcare professionals to coordinate client appointments and follow-ups.
Ensure that all administrative processes are conducted efficiently within the office environment.
Requirements
Strong administrative skills with a focus on organisational efficiency.
