Hours: Part-time, 18 hours per week Days: Monday, 1:00PM - 5:30PM Wednesday 8:00AM - 12:30PM, Thursday 8:00AM - 12:30PM and Friday, 8:00AM - 12:30PM No lunch break during these hours

Job Overview

We are looking for a compassionate, detail-oriented

Care Navigator

to join our team. In this vital role, you will support patients by helping them understand and access the care services and resources they need. This position combines strong administrative capabilities with empathetic patient engagement, ensuring individuals receive timely and appropriate support throughout their care journey. Full in-house training will be provided.

Key Responsibilities

Provide personalised assistance to patients in understanding and navigating healthcare services and available community resources. Accurately maintain patients records, care plans, and interaction logs through data entry and documentation. Schedule and coordinate appointments and care services, ensuring a seamless experience for patients. Respond to patient queries over the phone, demonstrating professional and courteous phone etiquette at all times. Liaise with healthcare professionals and external agencies to support patient access to services. Efficiently manage general office duties to support team operations and maintain a well-organised work environment. Utilise Microsoft Office applications for documentation, communication, and reporting tasks.

Skills & Qualifications

Proficient computer skills, including Microsoft Office (Word, Excel, Outlook). Previous administrative experience with clerical duties and data entry is beneficial, though not essential. Excellent organisational skills, with the ability to manage multiple tasks while maintaining attention to detail. Strong typing abilities for accurate and timely data entry. Professional phone etiquette and effective communication skills.
