About The Role



About the Role



Assessment & Care Planning

Complete detailed assessments for new residents, identifying needs and risks. Create personalised care plans that reflect residents' wishes, preferences, needs and life history. Review and update care plans monthly, or sooner if circumstances change.

Collaboration & Person-Centred Support

Work closely with residents, their families, and colleagues to develop truly person-centred care. Promote positive risk-taking, enabling residents to live fulfilling lives in a safe and supportive environment. Ensure care plans include meaningful input from clinical leads and reflect best practice in dementia care.

Monitoring & Compliance

Maintain accurate electronic and written records, updating in real time. Ensure care plans and assessments meet regulatory requirements. Monitor, evaluate and report on outcomes, completing monthly KPIs.

Safeguarding & Well-being

Assess and record mental capacity, following best interest processes when needed. Ensure Deprivation of Liberty Safeguards (DoLS) are applied for and reviewed in line with legislation. Support safe medication practices, ensuring protocols are robust and clinically approved.

Team Communication & Record-Keeping

Ensure information is documented clearly and shared with the wider team. Provide guidance that helps staff deliver safe, meaningful care. Contribute to creating a culture of dignity, respect, and person-centred support.

Required Criteria



Care experience in a senior role Experience in writing and reviewing person centred care plans and risk assessments Experience in assessing mental capacity and best interests and working Excellent communication and presentation skills Excellent IT skills Attention to detail Ability to work in a fast paced environment with excellent organisation skills Experience using digital care planning systems

Guild Care was founded in Worthing in 1933 by visionary volunteers dedicated to supporting vulnerable individuals in the local community. Originally known as Worthing Council of Social Service, the organisation focused on alleviating poverty, isolation, and health challenges during difficult times. Over the following decades, Guild Care consistently expanded its services, proactively responding to the community's evolving needs.


In the 1950s and 1960s, Guild Care established innovative day centres and residential care homes, significantly enhancing life quality for older adults and individuals with disabilities. Adapting continually, the charity later introduced specialised dementia care facilities and tailored services for those with learning disabilities.


Today, Guild Care supports over 3,000 people annually across West Sussex through a range of community-focused services. Remaining true to its founding values, the organisation is one of Worthing's largest employers, powered by almost 700 dedicated staff members and the essential contributions of more than 300 volunteers, all united in making a meaningful difference.
