Job Details





The successful candidate will be part of our growing Outreach team within HSCP Adult Supported Living Core & Cluster Service. You will be expected to promote social inclusion and offer opportunities to support people in their own homes with a range of physical and learning disabilities mental health, offending behaviour, substance misuse, acquired brain injuries, blood born virus etc.





You will support service users by increasing their social and independent living skills in a person-centred way that promotes the principles of social inclusion.





You must demonstrate good communication & organisational skills, Ability to manage and prioritise workload, Work with minimal supervision, have team working with Literacy and IT skills. Can lead and mentor those working with you, the role will include working as part of a team as well as lone working out in the community.





Duties include all aspects of personal care, must have the ability to undertake all Moving and Handling manoeuvres and tasks including the pushing and pulling of wheelchairs, and all aspects of C.A.L.M and Escape. Training will be provided upon completion of induction.





The role will include keeping accurate records of outcomes focused support planning and reviews for the people whom you support, ensuring changes in service user health and wellbeing, support plans, risk assessments etc. are recorded and communicated across the team.





You will work as part of a team liaising with colleagues to ensure an integrated, multi-disciplinary approach to service delivery.





Flexibility will be required in the way of days/hours are worked for each post dependent on the needs of service users. The successful candidate will work on a rota basis which includes days, evenings, weekends, sleep-ins, and wakened nights, including public holidays, Christmas and New Year.





The successful candidate will be expected to travel throughout Fife; travel expense including mileage and bus ticket claims is available.





Staff will be supplied with their own SOS Lone Worker Fob and works smart phone which they will be expected to always carry with them.





Enhancements for Unsocial Hours, Sleepovers and Public Holidays Auto enrolment onto the Local Government Pension Scheme (LGPS) With 32 annual leave days rising to 37 days after 5 years. The Person





Must have or be willing to achieve in the first period of registration, the Social Services and Healthcare SCQF level 7 (equivalent to SVQ3) or equivalent to meet Scottish Social Services Council registration.


You will have experience/awareness of National Care Standards service and SSSC Codes of Practice.


Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.


As this role requires SSSC registration, the successful applicant will have three months to apply for registration.


If appointed, you must start your application as soon as possible so you're registered within the required six months.


If you need to achieve a qualification to maintain your SSSC membership, information will be provided if you are appointed. You must ensure you complete the qualification within the specified timeframe.
