Towyn Capel Residential Home in Trearddur Bay are seeking a compassionate and dedicated Care Practitioner to join our team. The ideal candidate will provide high-quality care to individuals in care homes and home care settings, ensuring their comfort, safety, and well-being. Duties

Deliver personalised care according to individual care plans, ensuring the dignity and independence of clients are maintained at all times Assist with daily activities such as bathing, dressing, feeding, and mobility support Support clients with medication management in accordance with prescribed instructions Maintain accurate records of care provided using IT systems and documentation protocols Communicate effectively with clients, their families, and healthcare professionals to ensure seamless care delivery Support clients in maintaining their routines and participating in social activities where appropriate Contribute to the development and review of personalised care plans in collaboration with healthcare teams

Experience

Previous experience working within a care home or home care environment is preferred but not essential; training will be provided for suitable candidates Strong communication skills in English, with the ability to build rapport with clients and team members Ability to demonstrate empathy, patience, and professionalism when supporting vulnerable individuals Knowledge of care planning processes and health & safety regulations within a care setting is desirable

Pay: 12.25-13.00 per hour
