Are you passionate about improving the lives of individuals with dementia? Do you have the skills and expertise to provide high-quality support and guidance to both residents and care staff? If so, West Berkshire Council has an exciting opportunity for you to join our Adult Social Care Service as a Dementia Practitioner.
The Role
As a Dementia Practitioner, you will play a vital role in delivering specialist dementia care within the Notrees Care Home. Reporting to the Registered Manager, you will be responsible for:
Providing expert advice and support to residents, ensuring their individual needs are met
Working collaboratively with care staff to implement person-centred care approaches
Conducting assessments and reviews of residents' care needs
Delivering training and guidance to staff to enhance their understanding of dementia care
Collaborating with external professionals to ensure comprehensive care and support
The ideal candidate will possess the following skills and attributes:
Extensive knowledge and experience in providing dementia care
Excellent communication and interpersonal skills
The ability to work effectively within a multidisciplinary team
Strong organisational and time management abilities
A positive and compassionate approach towards residents and their families
This role is fixed term until February 2026 - Interim support (short term organisational change)
