Job Description

Job Purpose



The Scheduler/ Care Assessor plays a key role in ensuring the smooth operation of care services by managing staff rotas, scheduling care visits, and conducting assessments. This role requires strong organisational skills, attention to detail, and the ability to respond quickly to staffing and service needs while maintaining high-quality care for service users.

Key Responsibilities



Rota Management & Scheduling (Primary Responsibility)



Complete weekly rotas, ensuring appropriate staff allocation to meet care package requirements. Update care planning systems (e.g., CarePlanner) daily to support payroll and invoicing processes. Ensure adequate cover for all shifts, including arranging cover for absences. Participate in an on-call rota, responding to emergencies and going out on hands on covering shifts when necessary.

Shift Cover & On-Call Duties



Cover shifts as needed to maintain service continuity. Respond to urgent staffing needs by coordinating cover or stepping in as required. Provide emergency support as part of the on-call rota, ensuring service users receive uninterrupted care.

IT & Digital Care Management



Maintain and update care scheduling software to ensure accurate records for payroll and invoicing. Support the transition to new digital tools, ensuring efficient documentation and communication. Assist staff in using IT systems effectively for care management and reporting.

Care Package Management & Assessments



Conduct initial assessments for new clients, completing all relevant paperwork, including care plans and risk assessments. Serve as the first point of contact for clients and their families, ensuring they receive clear information about their care package. Liaise with social workers, community resource teams (CRT), and other professionals to ensure holistic care planning. Regularly review and update care plans, ensuring they continue to meet service users' needs. Upload all assessment and review information onto digital care management systems (e.g., CarePlanner, Nourish). Inform care staff of new clients and ensure they have all necessary information before starting care.

Person Specification



Essential:



Experience in domiciliary care or a similar care setting. Strong scheduling, rota management, and organisational skills. Proficient in using IT systems for care management and reporting. Ability to work independently and as part of a team. Registered with Social Care Wales (or eligible for registration). Full UK driving licence with access to a vehicle, able to drive to locations across Anglesey and Gwynedd. Willingness to participate in an on-call rota. Ability to think innovatively and provide solutions to challenges.

Desirable:



Experience using CarePlanner, Nourish, or similar digital care management systems. Knowledge of care regulations and compliance requirements.

Additional Information



Travel within Gwynedd and Anglesey may be required. The role requires flexibility to respond to urgent needs, including covering shifts when on call.
