The client is a 52-year-old gentleman who suffers the effects of an acquired brain injury.
The client requires effective support in order to maintain appropriate standards of personal independence, life skills and behaviour.
The client requires motivational support in order for him to access vocational and recreational activities.
The role involves supporting the client within his own home and out in the community.
The support worker role will involve you assisting with the client's rehabilitation and personal support. The role involves close cooperation with the client's family, Case Manager / Practitioner Case Manager, and any othim relevant professionals.
Implementing the care and support plan, rehabilitation exercises and guidelines.
Providing the Client with support enabling him to access a range of social, recreational and rehabilitation activities such as caring for his pets, going to the cinema, cafes and pubs.
Supporting and encouraging the client to live an independent life under the guidance of the Team Leader, and Case Management Team.
Reporting any issues or concerns to the Team Leader and or the Case Manager/ Practitioner Case Manager where required.
Supporting the client in ensuring his property is well maintained.
To carry out light domestic duties in his home as appropriate such as vacuuming, washing and ironing.
To act as a driver, using the client's own vehicle (mainly locally and further afield if required such as Ascot).
To provide daytime, evening, and overnight support on weekdays and weekends, as required within a 24-hour support package.
Availability to cover annual leave & sickness.
To undertake training and supervision as agreed by the Case Manager.
To ensure client confidentiality in all matters relating to the client.
Skills, Knowledge and Experience
A non-judgemental approach with the ability to see things from other people's perspectives and be respectful of their lifestyle choices.
Calm communication style with clear pronunciation when speaking.
Understanding of the 'enabling approach' and the ability to judge when direction needs to be offered to the client.
Sense of humour and an understanding of appropriate boundaries with a mature, confident and professional approach.
To be able to integrate an understanding of brain injury - displaying empathy but with non - patronising approach.
Ability to follow the support plan, The Case Manager/ Practitioner Case Manager/ Team Leaders guidance and instruction as well as the ability to accept feedback.
To display an understanding that the client's workplace is also the client's home.
Ability to identify and manage potential risks to safeguard the client.
Sensitivity and discretion necessary to working within a client's own home.
Ability to absorb complex information and training provided by the Case Manager / Practitioner Case Manager and any other relevant professionals to demonstrate accurate implementation of instruction and guidelines.
Effective team working skills.
Excellent written and spoken communication skills.
Behaving within the good practice guidelines and standards of consent outlined in 'The Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England'.
