Job Summary:

Relatives and friends of residents, representatives of external partner agencies.

Key Responsibilities:



1. Quality of Care



1.1. Contribute to person centred assessment planning, implementation and review.

1.2. Put into practice person centred care including for residents with dementia.

1.3. Support independence in the tasks of daily living.

1.4. Promote equality, diversity and inclusion.

2. Leading and Managing Staff



2.1. Share in the safe selection, recruitment and retention of staff to develop a quality service

2.2. Take responsibility for the continuing professional development of yourself and staff you are responsible for by ensuring the implementation of the Salvation Army's Learning and Development objectives for the Care Staff within your team, including induction.

2.3. Supervise, appraise and support a small team of care assistants to ensure that the home is meeting the individual needs and preferences of residents.

2.4. Where necessary, participate in investigations related to the disciplinary process where conduct, attitude or quality of work is inconsistent with the goals and objectives of the home.

2.5. Promote person centre thinking and planning.

2.6. Promote active support.

2.7. Ensure that the policies and procedures for the conduct of staff are adhered to by implementing the Salvation Army's HR Policies and Procedures.

3. Leading the Shift



3.1. Lead the Care Staff when you are the Duty Team Leader to support a quality service delegating tasks and supporting staff with their duties.

3.2. Divide the care of residents between staff to ensure person centred care.

3.3. Facilitate the admission process for residents including the allocation of a key worker.

3.4. Implement the admissions process for new residents ensuring that they feel welcomed to the home.

3.5. Allocate break times to ensure continuity of care.

3.6. Where necessary investigate and or participate in investigations relating to Complaints about the Care of residents implementing The Salvation Army's Complaints procedures and all relevant Safeguarding Procedures.

3.7. Where necessary escalate any issues of concern by informing and consulting with more senior management.

3.8. Undertake return to work interviews in compliance with The Salvation Army's absence procedure.

3.9. Participate in and undertake the handover of information at the beginning and end of shifts.

3.10. Administer medication to residents and monitor the effects, being accountable for the safe keeping and storing of medication.

4. Providing Care



4.1. Create implement and review care plans with residents.

4.2. Administer medication to residents and monitor the effects, being accountable for the safe keeping and storing of medication.

4.3. Ensure that your own actions support the care protection and well being of residents.

4.4. Arrange appointments and visits for residents and facilitate safe transport.

4.5. Facilitate residents to participate in recreational and development activities.

4.6. Provide food and drink to residents and if necessary assist them in eating and drinking.

4.7. Help address the physical comfort needs of residents.

4.8. Support residents with their personal care needs and to manage continence.

4.9. Provide support for mobility and contribute to moving and handling residents.

4.10. Support residents to monitor their own health care needs.

4.11. Support residents who are distressed.

4.12. Support residents at the end of their life

5. Recording



5.1. Observe, monitor and record the condition of residents.

5.2. Communicate with and complete records for residents including contributing to care plans.

5.3. Receive and pass on messages and information.

6. General



6.1. Support the health and safety of yourself and residents reporting any concerns as appropriate.

6.2. Take a lead role for some aspect of care practice as required.

6.3. Participate in the on call duty rota.

6.4. Be available for Supervision and Appraisal.

6.5. Develop your knowledge and practice.

6.6. Undertake other duties as deemed necessary to ensure the smooth running of operations and fulfil The Salvation Army's mission.
