As a Community Care Team Leader, you will play a pivotal role in ensuring the highest standards of care for our service users within their own home. You will be responsible for supervising the care team, driving quality improvements, and implementing effective care plans that cater to the needs of our service users. Your leadership will foster a supportive environment that promotes both service users wellbeing and team collaboration.
Responsibilities
Supervise and support the care team in delivering exceptional care to service users.
To be responsible, professional and effective when on call every 4 weeks when the main office is shut, additional payment for on call duties.
Develop, implement, and review individual care plans to meet the unique needs of each service user.
Communicate effectively with service users, families, and staff to ensure clarity and understanding of care requirements.
Drive initiatives that enhance the quality of care and improve service users satisfaction.
Conduct regular assessments of service users needs and adapt care strategies accordingly.
Assist the Care Manager and Co Ordinator with rostering care.
Ensure compliance with all regulatory standards and organisational policies within the company.
Skills
Minimum of 1 years care experience in a senior or team leader role.
Strong leadership skills with the ability to motivate and guide a team in a dynamic environment.
Excellent communication skills, both verbal and written, to effectively engage with service users, families, and colleagues.
Experience in senior care or a similar setting is highly desirable.
Good IT skills for maintaining records and documentation related to care plans, rostering care.
Ability to drive change and implement best practices within the team.
Competence in supervising staff while fostering a positive workplace culture.

This is a community based position covering Tavistock and surrounding areas so your own transport is essential and a full driving license.

Pay: 13.10-13.20 per hour
