Main Responsibilities:
Provide safe and compassionate care and support that is person-centred.
Perform personal care tasks as outlined in the client's care plan.
Create and maintain good communication with clients and other members of the support network.
Establish and maintain effective working relationships with clients.
Document, monitor and report any changes to care managers.
Assess care plans versus actual provision.
Assess risks and needs, reporting to the care manager.
Participate actively in clients' care team meetings.
Adhere to all Eclectic Care Ltd policies and ensure new staff are inducted into clients' care packages and follow procedures.
Offer companionship and emotional support to enhance the well-being of clients.
Ideal Candidate Must Be Able to Support Clients With:
Attending appointments and social activities.
Helping with dressing.
Laundry and light cleaning.
Meal preparation.
Medication management.
Personal care, including washing and bathing.
Shopping.
