Job Overview


The outreach worker will be part of the Carers Hwb team and will work to engage with carers who are unable or don't want to access the city centre base.

They will focus on trying to engage carers who are underrepresented such as those from ethnic minorities, male carers and those served by poor public transport links. The Outreach worker post is funded as a part of a National Lottery Community Fund project.

1. ACCOUNTABILITY

The Outreach worker will be employed by the Swansea Carers Board of Trustees. Line management for this post is through the Hwb Coordinator.

2. PURPOSE OF THE POST

Engage with underrepresented carers

Undertake Hwb initial assessments with those unable or not wanting to attend the Carers Centre

Participate in community events to promote the Hwb and the services of Swansea Carer Centre

Manage and support a caseload

3. Service Planning and Delivery:

Identify and Engage: Identify individuals or groups in need of assistance and engage them to understand their needs.

Provide Information and Assistance: Provide information about available services and help individuals access these resources.

Assess Needs: Conduct assessments to determine the specific needs of individuals and communities.

Referral Services: Make referrals to appropriate services and assist with necessary paperwork.

Develop Outreach Programs: Create and implement outreach initiatives raise awareness of Swansea Carers Centre Services across communities in Swansea

Volunteers - train and support volunteers to be part of the outreach service

Collaborate: Work with other health and social care professionals to best meet the needs of the carers.

Follow-Up: Conduct follow-ups to ensure individuals are receiving appropriate services.

Record Keeping: Maintain confidential records of carer interactions and services provided.

Advocacy: Advocate on behalf of individuals and communities to highlight needs and gaps in service.

Crisis Intervention: Provide crisis intervention when necessary.

Educational Workshops: Conduct workshops to educate communities about health issues, prevention, and available resources

Community Activity - participate in various community activities

4. Good Practice, Monitoring & Reporting

To ensure the project is meeting agreed outcomes and milestones identified in the funding bid.

To provide monitoring information and report on the project to the funder

To prepare documents (reports/evaluation and statistical data) with accuracy, and submit to the Board of Trustees and funders as required.

To contribute to the development and implementation of the organisation's quality assurance systems.

To take part in team planning, monitoring and evaluation.

To participate in continuing professional development through training and education.

5. General

To participate as a member of the staff team.

To work at all times within the philosophy and policies of Swansea Carers' Centre.

To comply with the Health & Safety at Work Act 1974 and with Swansea Carers' Centre policy, paying particular attention to the reporting of dangerous situations.

To respect the personal choice and lifestyles of colleagues, carers and people with care needs, ensuring that Equal Opportunity and Diversity principles are applied at all times.

To undertake any other duties that may be considered commensurate with the level of the post

Other duties

To take responsibility for other duties relevant to the purpose of the role as requested by the line manager.

To provide regular progress reports for the Hwb Coordinator and Board of Trustees.

Person Specification

Essential Knowledge, Skills and Experience, Relevant Experience

Understanding of carers' issues.

Abily to work on one's own initiative and without close supervision

Able to develop strong working relationships

Previous experience of assessing individual needs and producing an action plan

Demonstrable local knowledge of third sector and health and social care

Experience of community engagement

Use of MS Office 365 and CRM databases

Ability to develop and maintain effective working relationship with colleagues and Stakeholders

Personal Competencies

Essential

Car owner/clean driving license.

Desirable

Ability to read and write in the Welsh language

Knowledge of the third sector

Job Type: Part-time

Pay: 19,146.40 per year

Expected hours: 28 per week
