We are seeking a dedicated Case Worker to join our team. The successful candidate will be the primary point of assistance for the residents of our supported accommodation, building trusting relationships through regular, proactive engagement. This will involve frequent support meetings, facilitating house meetings and conducting property inspections to ensure a safe and positive living environment.
You will work intensively with vulnerable adult service users, many of whom have multiple and complex needs. A key aspect of the role is managing your own caseload and working autonomously to support residents in liaising with external agencies including medical services.
Duties and Responsibilities
Serve as the primary support for assigned residents, working closely with them in their home environment through regular, scheduled one-to-one support meetings.
Facilitate and lead regular house meetings to foster a positive community, address communal issues, and promote resident engagement.
Conduct regular inspections of the houses to ensure health, safety and well-being standards are maintained, working with residents to create safe and dignified living space.
Engage face to face with our guests (service users) to build trust and then support them in liaising with other agencies (Leicester City Council, medical services, other relevant referrals)
Support the team in the provision of services
Be responsible for ensuring our rules are adhered to, including banning guests where necessary.
Engage in morning outreach in conjunction with the city council, with the aim of breaking down barriers to create trust.
Keep accurate, complete, and up to date records of all support work undertaken, ensuring records are kept safe and confidential in accordance with Data Protection Legislation.
Monitor, measure and report on outcomes, contributing to reports to the Board of Trustees and Funders.
Attend meetings with strategic partners, project partners and other external agencies as appropriate.
To attend and actively engage in weekly staff meetings, mandatory and ad hoc training courses, supervision and appraisals
Adhere to all organisational policies and procedures at all times.
To undertake any other relevant duties as deemed necessary.
Qualifications
Previous experience in a similar role (or can show transferable skills) is essential
Have a good knowledge of the support services available in the City of Leicester
Have an understanding of conflict management and de-escalation techniques
Excellent report writing, administration and IT skills
Ability to maintain professional boundaries
Good interpersonal skills, including active listening, clear and thorough information sharing and verbal communication skills
Ability to prioritise, organise and manage workloads to meet deadlines and to work flexibly with guests who have high needs
Ability to cope with the emotional demands of the role
Ability to work independently and as part of a team
